What is SI Joint Dysfunction?

The sacroiliac (SI) joint links the pelvis and lower spine and has the primary function of providing stability for the pelvis and bearing the load of the upper body when you stand, walk, and shift weight.

SI joint dysfunction generally refers to pain in the low back and buttock region caused by abnormal motion in the sacroiliac joint. This is thought to cause chronic low back pain, groin pain, and buttock pain, all of which can negatively impact one’s quality of life.

What Causes SI Joint Pain?

Hypermobility of the SI joint describes too much movement, and this excessive motion may inflame and disrupt the joint and surrounding nerves. Hypomobility, often caused by arthritis, occurs when the joint has too little movement, which causes the bones to rub against one another, creating extreme pain and may inhibit mobility.

Causes of hypermobility and hypomobility of the SI joint include:

  • Traumatic event or impact (fall, misstep, lifting, twisting, car accident)
  • Degeneration
  • Postpartum
  • Anatomic abnormalities
  • Previous spine surgeries
  • Infection
  • Arthritis

How Do I Know If I Have SI Joint Dysfunction?

SI joint dysfunction is a common source of pain in patients with low back pain. While low back pain can be an acute, short-term problem for some, others experience chronic, long-term symptoms that can limit mobility and affect their quality of life. Common signs and symptoms include:

  • Dull, unilateral low back pain and stiffness
  • Buttock pain, may radiate to thigh/ groin
  • Pain that becomes worse while doing activities like climbing stairs or hills
  • Feeling of leg instability or gait changes
  • Muscle tightness and tenderness in the hips or buttocks
  • Disturbed sleeping patterns and sitting patterns due to pain
  • Temporary relief from SI joint injections

Oftentimes, SI joint pain is misdiagnosed as a spine condition. This misdiagnosis frequently results in unnecessary surgery, which is why it is crucial to have an accurate diagnosis for SI joint dysfunction.
